Please drop off completed forms in a sealed envelope. Payment information and due dates are listed in the parent handbook.
In addition to the registration forms, all campers must have a copy of immunization record and a recent physical report on record before attending camp.
In addition, if your child needs medication while at camp, parents must complete a consent form and must have a doctor’s written order for medication administration at camp.
